REFERRALS
If you need to get a referral for a specialist or ancillary service, please let your doctor know and he/she will send an electronic message to our referrals department. The Referrals Department will contact your insurance company to obtain authorization (if necessary). After this is obtained, our staff is happy to assist in referring you to a specialist in the area and sending the requested records to them. Someone from our Referrals Department will call you with the specialist information along with any appointment date and time(s). Some specialists do require that the patient schedule their own appointments.
- FWC is not responsible if you are referred to a non-preferred or out of network provider, but our staff will make every effort to locate participating specialists on your insurance plan.
- For the best outcome regarding your referral, you should review and verify your insurance plan benefits, coverage and limits prior to your specialist appointment.
- Unless urgent, please allow 7-10 working days for our referrals department to process non-urgent referrals.

New Patient Registration & Financial Policy form
All new patients must complete this form to set up an individual account with Family Wellness Center. Bring completed form along with your insurance card with you to your first appointment.
All NEW adult patients must complete this form and bring it to their first office visit appointment.
Children & Teens Under Age 18 form
A parent of a NEW pediatric patient should complete this form and bring it to their child’s first appointment at Family Wellness Center.
