What to Expect at Your First Visit
You are a new patient? Welcome!
When you schedule your first appointment, we make sure you will get our welcome packet as soon as possible. Our staff can mail it to you, or you can download it here. If you choose the download option, we will send you a reminder postcard.
Two new patient forms are part of the welcome packet. All new patients need the “New Patient Registration & Financial Policy” form. Adults and patients under age 18 have their own “New Patient” form as well that you can download here. Please fill them out before you come for your first visit!
Also, please arrive 15 minutes prior to your appointment time – we have plenty of free parking. Bring with you:
- The completed new patient forms.
- Your insurance card. We will scan it into your electronic record.
- Prior health records if you have not had them sent already by your previous provider.
- Copayment or deposit if your insurance requires it. We participate in many insurance plans and networks. You need to check with your insurance company if you are not sure about your co-payments or coverage.
When you arrive, our receptionists will greet you and get you checked in. We have a comfortable reception area with a special space for children, but we schedule carefully so that you won’t spend much time there.
When we are ready to bring you to the examining room, your medical assistant will escort you back, take your vital signs, and do whatever else we can to prepare you for your visit with your provider.
